Managing Form Types (Form Templates)

What are form types?

Form Types in Formulayt allow administrators to maintain global governance and standardization across your form estate while reducing technical overhead. Form Types are the templates that define the structure, behavior, and validation rules that ensure consistent data capture and compliance across all your forms.

Creating a new form type

To create and edit Form Types:

  1. Navigate to Setup / Form types on the menu bar

  2. Click Create a form type

  3. Choose a name for the form type (we recommend establishing a globally agreed taxonomy)

  4. Choose a form behaviour (Standard, Progressive or Multi-Step)

  5. Determine where the form type will submit to (default submission routing or choose an alternative)

Adding and managing fields

Now you can start adding fields to your form type, select the Fields tab, then click Add fields.

When adding fields:

  • Select fields in the order you want them to appear on the form

  • Planning the field order in advance will save you time

  • You can always rearrange them later if needed

Setting field properties

To configure basic field properties:

  • Make a field mandatory or optional using the toggle switch to the right of the field block

  • Change the field order by clicking and dragging each field on the left hand side of the field row

  • Arrange fields in the sequence you want them to appear to users

Advanced field settings

You can access more advanced settings by clicking the cog button for each field.

Advanced display options include:

  • Making field display switchable so gate editors can choose whether to show the field

  • Making the mandatory status switchable so gate editors can choose whether to enforce completion of the field in a gate

  • Hiding the field if it is pre-populated with a value

  • Setting the field to be 50% (to allow two column layouts in forms)

Navigating to the edit field area

You can also click the pencil button for each field to navigate to the Edit field area for that field.

Viewing form type usage

It is also possible to see where a form type has been used. Click the Form type usage button to go to a report for that form type.
